CONSENT FOR USE OF PERSONAL INFORMATION

I authorize Muslim Athletic Association to collect and use personal information about me (or my child/ward if applicable) for the purpose of receiving communications from Muslim Athletic Association; and the disclosure of my name and address (or my child/ward’s name and address if applicable) to the respective venue(s) for the purpose of securing fields/venues and no other purpose. *We do not sell or distribute your personal information to any other third party not listed herein.*

ACCEPTANCE OF TERMS AND CONDITIONS

In consideration of the acceptance of my membership in Muslim Athletic Association, I, the participant, agree as follows: 1. I am aware of Muslim Athletic Association’s policies, rules and regulations and agree to abide by them and to be bound by them 2. I accept all liability for any damage to the playing equipment caused by my careless, negligent and/or improper handling. 3. I understand that Muslim Athletic Association may take photographs or videos of participants during programs ( we will typically give you a heads up when we will have a photographer come in).

WAIVER AND RELEASE OF LIABILITY

Muslim Athletic Association their directors, members, employees, coaches, volunteers, officials, participants, agents, sponsors, owners/operators of facilities, and representatives are not responsible for any injury, damage or loss of any kind suffered by a Participant during, or as a result of, any program, activity or event, caused in any manner whatsoever including, but not limited to, the negligence of Muslim Athletic Association. I hereby acknowledge that I am aware of the risks and hazards associated with or related to any program offered by Muslim Athletic Association. In consideration of Muslim Athletic Association allowing me to participate as a Participant, I agree: to assume all risks arising out of, associated with or related to my participation; to be solely responsible for any injury, loss or damage that I might sustain while participating; and, to release Muslim Athletic Association from liability for any and all claims, demands, actions and costs that might arise out of my participation, even though such risks, injuries, loss, damage, claims, demands, actions or costs may have been caused by the negligence of Muslim Athletic Association

CODE OF CONDUCT FOR PLAYERS

Muslim Athletic Association has a zero tolerance policy for the following actions: Disrespect of teammates, opponents, officials, staff and spectators through the use of inappropriate language or gestures; Lewd behavior on and/or off the field, Physical altercations, Consumption of alcohol, tobacco, illegal substances and public urination on any fields used by Muslim Athletic Association

REFUND POLICY *

Muslim Athletic Association adheres to a strict refund policy and this policy applies to registration fees paid either online or in person: There is no refund of any fees if a player/team/parent is suspended or expelled from Muslim Athletic Association for violation of discipline policies or codes of conduct. There is no refund for voluntary withdrawal from the program. There will be no refund if a player or parent disagrees with the player's coach, team or tier placement. A refund will be issued if Muslim Athletic Association is unable to field a team or provide a reasonable program alternative.
